Benefits to PCOs We enjoy a close working relationship with PCOs and DAATs and can offer a whole host of benefits through our programmes. As an established and trusted pharmacy, we know that our methods can deliver great results. • Our extended opening hours and large geographical cover gives better access to services and pharmacists • We provide a robust, integrated service to patients • Patients have easy access to treatment close to where they live giving them a personal service and environment of mutual trust • We encourage patients to seek help and advice when needed • Our free information packs contain advice on health risks, diet and lifestyle Our experience We provide a successful bespoke alcohol screening and brief advice service in Birmingham for three PCTs; Heart of Birmingham, Birmingham East and North and South Birmingham. The aim of the service, launched June 2009, was to minimise the risk of long term alcohol related health problems, in particular hospital admissions, by raising awareness of sensible drinking and helping people to understand their drinking habits. Working in close collaboration, we have produced a bespoke service and leaflets with details of the service and advice on safer drinking and how to avoid long term health risks. We collect local activity data and provide reports to the PCTs. The service is managed locally so any issues can be quickly resolved. It also means that the DATs and PCOs have a single point of contact for the service. NHS Health Checks Lloydspharmacy aims to demonstrate that community pharmacies are ideally placed to offer public health services and support the Department of Health’s national vascular programme - NHS Health Check. A perfect example of this is our NHS Health Check programme, which helps to identify individuals with an increased cardiovascular disease (CVD) risk, allowing us to encourage early preventative treatment and suggest positive lifestyle changes. The health check also includes diabetes, blood pressure and cholesterol tests.
This innovative new service fulfils a number of objectives, including meeting local needs, reducing health inequalities and supporting delivery on this key government priority. Our clearly defined referral criteria are developed in agreement with local clinical leads and can be based around: • Cardiovascular risk assessment score • Blood pressure • Total cholesterol • TC/HDL ratio • Blood glucose Other risk factors are also considered such as age, gender, ethnicity, BMI, waist circumference, smoking patterns, alcohol intake and exercise regimes. Benefits to PCOs Our programme is flexible, so can be tailored to meet the needs of individual PCOs, using protocols developed in line with the national service specification. PCOs will see a number of benefits as a direct result of taking part in the programme, including: • Achieving target number of NHS Health Checks • Improved service development • Increased likelihood of strategic partnerships • Reducing health inequalities • Comprehensive training • Better patient engagement Our experience Research commissioned by the Birmingham Health and Wellbeing Partnership established that a cardiovascular screening service was needed in the area. Lloydspharmacy successfully tendered for the service, and the Birmingham Health and Wellbeing Partnership Cardiovascular Mass Screening Service was launched.
Patients were screened in the evenings and at weekends, a key factor in the success of the service as most of those involved worked during the day. The simple, yet highly effective, method of telephoning patients to invite them to screenings meant that attendance rose to 80% of those invited (as opposed to 20% when just a letter was sent). Our results: • More than 10,000 males over the age of 40 were screened • 65% of those attending were referred • Of these: • 36% were identified as having a high CVD risk • 30% were referred due to elevated blood pressure levels • 35% were referred due to elevated cholesterol levels • 18% were referred due to high blood glucose results Satisfaction surveys gave positive results: • 98% would recommend the service to a friend • 97% would come again • 76% plan to make changes as a result of the screening • 99% were satisfied with how the tests were conducted • 99% were satisfied with their explanations • GPs were also very positive, as the service targeted the hard-to-reach population who don’t usually visit the surgery Our success has resulted in further activity supporting Manchester PCO and Hull PCO. We also continue to offer our Cholesterol and Heart Check service in over 600 pharmacies. The aim of the service, launched in 2002, was to minimise the risk of blood-borne infections in injecting drug users, by improving the provision and management of clean needles and syringes. Working in close collaboration with Staffordshire County and Stoke City Drug Action Teams (DATs), we have produced bespoke packs containing a local service leaflet with details of the service and advice on safer injecting and how to avoid an overdose. We collect quarterly local activity data and provide reports to each DAT. The service is managed locally so any issues can be quickly resolved. It also means that the DATs and PCOs have a single point of contact for the service, and don’t have to worry about stock control or pack distribution. Lloydspharmacy’s involvement also means we can help to engage more drug users in treatment programmes, by offering a range of services (which in turn can help achieve local and national targets). Sexual Health As part of our commitment to offering public health services through community pharmacies, Lloydspharmacy has developed a series of innovative solutions targeting sexual health issues. Through close partnership working, we have supported PCOs in developing and piloting their sexual health initiatives. These cover areas such as oral contraceptives, supply of EHC (emergency hormonal contraceptives), chlamydia screening and treatment. Furthermore, our services in this area also include impotence treatment. At Lloydspharmacy we take a joined-up approach to sexual health. We have a pharmacy network in the heart of communities, proven successful web-based services and strong collaborations with key industry experts, all of which allow us to offer targeted solutions through a variety of different channels. Benefits to PCOs We can assist PCOs by supporting them in their sexual health initiatives in a number of ways. We are here from initial engagement through to end use; we can help to raise awareness of the services even in hard-to-reach areas, and increase uptake of the service through a number of different channels. We provide tailored packages of appropriate services; for example, targeting for chlamydia can be achieved through us going out into the community, mass screening events, community pharmacy-based services, mail-out services and web/SMS request services, while contraceptive services can be delivered via a community pharmacy. PCOs will benefit from: • Comprehensive solutions which don’t involve development resource • A joined-up approach leading to seamless care • A wide range of channels, allowing access to hard-to-reach groups • Support in reaching NHS targets Our experience We have a wealth of experience in supporting sexual health services, and this is demonstrated by the real results we’ve generated. For example, our pharmacy in Fallowfield has been acknowledged for its expertise in EHC and chlamydia screening and treatment, and is now also piloting the provision of oral contraceptives. We have also been working in partnership with sexual health expert Dr Thom. Through this online collaboration, we have developed private services and tested an alternate method for client engagement, which can be easily adapted to fit individual PCO requirements. The number of patients receiving oral anticoagulation therapy is growing by about 10% each year making it essential to find more capacity to ensure these patients receive the best level of care. Managing this additional demand will place a massive strain upon both GP and hospital-based clinics. Devolution of INR clinics to community pharmacy is a practical solution and one which releases valuable resources, allowing GP surgeries and primary care organisations to focus their attention on other areas of patient care. Working together with key ambulance trust staff, Lloydspharmacy are able to develop a bespoke solution to meet our client’s needs. This service will minimise the time required internally by ambulance crews to maintain their drug stock and also to increase safety. We have developed a flexible service whereby: • Drug requirements are contained within an outer carrying bag in four separate colour coded bags, to allow for quick and easy location of required drugs. • Each bag contains paperwork recording all drugs used and breakages and as such ensures drugs needing to be replaced can be easily identified. • Once the stock within any bag has reached a level requiring replenishment it is exchanged for a new complete bag at the local ambulance station. • A weekly service is provided to replace used bags. • All used bags are checked fully and stock replenished before being tagged as complete. • The use of drugs, damages, stock discrepancies, batch number recalls etc. are all maintained by Lloydspharmacy to allow for full audit of the service. Being a community pharmacy, we are in a strong position to support PCTs in engaging with target communities in a number of ways: Benefits to PCOs • We are able to work collaboratively with other pharmacies to deliver cost effective distribution of test kits and treatment of positives across a wide network of community pharmacies. • Located within close proximity of most people, we provide ready access to chlamydia screening and signposting to specialist services. This service is a complete package including NAAT-based laboratory testing and communication of results. • Direct mail-out is another opportunity to reach large numbers of under 25s and our partnership with our online GP service is able to deliver a complete package of kit distribution, laboratory testing, results management, partner notification and treatment. Our experience • We already have wide ranging expertise in delivering chlamydia screening and treatment for many PCTs nationwide as part of an enhanced service offering • Lloydspharmacy has been involved in a number of outreach initiatives to deliver mass screening events such as our systematic cardiovascular screening events in Birmingham which have screened 10,000 people in the target audience. One in eight acute hospital admissions are due to COPD, and it accounts for over one million hospital beds occupied in England at a direct cost of over £500 million pounds for healthcare. Screening: Lloydspharmacy are developing a COPD screening service that will include: • Screening all smokers over 35 years of age • History taking including smoking history and symptoms • Measurement of airways obstruction using a screening device - spirometer • Referral to GP where a risk has been identified Management: Lloydspharmacy can work with local practices to ensure those who are diagnosed with COPD are utilising their prescribed medicines appropriately. This service could include: • Dyspnoea score • Inhaler technique check • Home visits for those with severe COPD receiving oxygen therapy and who are housebound • Review of management plan, if applicable

Pharmacy-led minor ailment schemes can help to address this issue. Evidence from existing minor ailment services demonstrates shifts in the minor ailment consultation workload from GP practices to community pharmacy. • GP minor ailment workload fell from 8.9% at baseline to 6.6% during the intervention • GPs felt that they were dealing with fewer minor ailments, most notably fewer prescription requests Lloydspharmacy has developed a comprehensive minor ailment service and we can work in partnership with primary care organisations to deliver the service across their locality. It works by gradually reducing the number of cigarettes smoked until the person is able to stop completely. The service has been developed using Prodigy guidelines and is intended to compliment the local NHS services already being provided by many of our pharmacies across the UK. Our staff are extensively trained according to the Health Development Agency standards, therefore minimal training is required in order to tailor the Lloydspharmacy service to meet the local needs of primary care organisations. Type 2 Diabetes Early Identification Service Lloydspharmacy offers a comprehensive risk assessment and blood glucose testing service. This service aims to raise awareness through effective health promotion, encourage positive adjustments to lifestyle and helps identify people who may have undiagnosed Type 2 diabetes. Our Experience The service started in Autumn 2002 as a pilot within Glasgow and was rolled out across the whole Lloydspharmacy estate during 2003. We have completed over 1.25 million screenings since the launch in 2003, and approximately 60,000 customers have required referral to their GPs for further investigation and treatment. Currently the Lloydspharmacy national identification service uses fasting blood glucose as an indicator of diabetes risk. An alternative service has been developed using HbA1c results for identifying diabetes. The diabetes screening service involves the initial test, counselling and referral to the patient’s GP if necessary. This service is provided in conjunction and with support from Diabetes UK. The service has received widespread recognition and has won the People in Retail Award for Customer Service Initiative in 2006 and the Diabetes UK “Raising Awareness Award” on two occasions. Management of Type 2 Diabetes Helping people to understand their condition and the reason their medication has been prescribed is important if maximum benefit is to be gained from treatment. This service can incorporate HbA1c measurement which allows the pharmacist to identify how well blood glucose has been controlled, along with a simple medication review. Good blood glucose control is important if complications of diabetes are to be avoided. Providing patients with information through the use of new technology, such as mobile phones, allows patients to take better control of their condition. Lloydspharmacy has worked with Oxford University and t+ medical Ltd to develop to t+ diabetes service. The t+ diabetes service can make it easier for patients to stay confidently in control by helping them to develop a better understanding of what affects their blood glucose levels, providing immediate feedback to the patient’s mobile phone whenever a reading is taken. These services can be tailored to meet local needs. Vaccinations In partnership with DrThom who are registered with the Care Quality Commission, Lloydspharmacy offers a vaccination service, giving quick and easy access to a variety of vaccinations including travel and cervical cancer. Whether visiting one of our pharmacies or using a computer elsewhere, patients interested in this service are able to complete an online medical questionnaire which has been specifically developed by DrThom. The responses to the questionnaire are reviewed by doctors employed by Dr Thom and if appropriate a private prescription is issued to Lloydspharmacy to dispense the prescribed vaccinations at one of our participating pharmacies. Our pharmacy teams are specifically trained to meet the requirements of the Healthcare Commission and have also been approved by Dr Thom. Their training includes all elements of vaccine administration and how to respond in the unlikely event of an anaphylaxis reaction. Our pharmacies have a private consultation area with all the necessary facilities to deliver a vaccination service. Furthermore, all participating pharmacies are issued with a Standard Operating procedure (SOP) to ensure the service is conducted in a consistent, professional and accurate manner. Records of all administered vaccines are sent to patients GPs to ensure practice records are updated.
