Moving care closer to home

Discover our range of out of hospital clinical services.

We’ve developed new services that treat patients who require traditional hospital treatments in the local community. These services ensure that the right care for each patient is delivered by the right professional and are especially useful in NHS Trust areas with nursing shortages.

Based in the local community in a dedicated facility, this service is an infusion suite in the heart of the community, offering patients the administration of infusion therapies, as well as Subcutaneous and IM therapies, screening, phlebotomy and health checks.

We administer sub-cutaneous injection treatments administered by nurses and specialist-trained pharmacists in a local LloydsPharmacy. This offers a convenient location for patients and transforms capacity for hospitals. Patients can access healthcare professionals who can administer the medicines and support with condition management and follow up care.

This service is a mobile infusion suite in various locations delivered in the heart of the community. Like the healthcare centre it offers patients the administration of infusion therapies, as well as Subcutaneous and IM therapies, screening, phlebotomy and health checks.

Improving patient outcomes.

Getting the product to the patient is only one part of the solution. Ensuring patients continue to take their medicines on time and in the right way is a huge challenge.

It is important to ensure that patients feel confident in taking their medicines and understand the importance of taking them as prescribed. Having a pharmacist or nurse-led patient support programme in place greatly improves adherence to medicines. Our dedicated team develop programmes that are tailored to the attitudes and beliefs of the patient.
